I've waited in Alexa's office, placed a coint under the chandelier, Alexa stopped and piked it up, I shoot the chain, she is crushed, accident kill, silent assassin rating still up, everything's perfect - challenge not unlocked. Tried twice on Pro and Master levels, disguised or not, still nothing.
